furnished by the Department of Public Safety.
B. Every application for a driver's license shall state the full name,
date of birth, sex and residence address of the applicant, whether the
applicant is deaf or hearing impaired, the license plate number and
state by which the license plate is issued for up to two (2) vehicles
owned by the applicant, and shall briefly describe the applicant. The
application shall also state whether the applicant has previously been

shall be denied unless the applicant submits an affidavit to the
effect that the applicant is not a resident of this state. The
Department of Public Safety and the Oklahoma Tax Commission shall
promulgate rules to administer the provisions of this subsection.
